Enhancing the Usefulness of the Psychosocial Assessment of Candidate Transplant Scale for Kidney Waitlist Patients Rasch Validation Study

Abstract
Purpose:To examine the psychometric properties of the Psychosocial Assessment of Candidate Transplant (PACT) scale using a Rasch model analysis in Korean deceased donor kidney transplantation candidates.

Methods:This methodology study was a secondary analysis of the 2017-year data from a parent study at an organ transplant center in Korea. 157 kidney transplant candidates were analyzed. We conduct recalibration and Rasch validation of this scale to enhance the clinical utility of the precise tool in transplant fields.

Results:The R1c statistic and Anderson test results showed that the model fit was stable, and both the outfit and infit
statistics were between -1.94 and 1.07. All the items fit the Rasch model. The discrimination of the items was good. The difficulty was between -0.22 and 2.27, reflecting acceptable levels. The person-item map indicates that suitable for identifying patients with relatively high-risk candidacy.

Conclusion:We revised and validated the PACT¡¯s items as a part of a management aid to help candidates. Assessment using the convert table can improve the reliability of the tool without the need to modify its original response scoring. We provided a practical and clinically useful tool for kidney transplant candidates. It can help transplantation professionals in assessing kidney candidates¡¯ psychosocial candidacy to promote successful post-transplantation outcomes.
